1ST VISIT & INTAKE PAPERWORK
As a new patient we will have you fill out some intake paperwork at your 1st first visit. We ask that you arrive 15 mins early to allow time to complete paperwork.
INSURANCE
Our office does not accept assignment from insurance companies. We will, on request, issue you a receipt that you may turn into your insurance company if you wish to make a claim for reimbursement (to be made directly to you). We do not submit paperwork directly to insurance companies.
PAYMENT METHODS:
We accept cash, checks, HSA cards and major credit cards.
Policies
CANCELLATION & SCHEDULING POLICIES:
We require 48 hours notice BY PHONE ONLY to cancel an appointment without charge.
Missed appointments (not canceled or rescheduled at least 48 hours in advance — BY PHONE ONLY. NOT BY EMAIL!) will incur a missed appointment charge.
Our Front Desk staff makes sure that you are notified and reminded of your appointments by email and text message. It is each patient's individual responsibility to keep track of the correct date and time of all your scheduled virtual visits and to log in on time or a few minutes early for your appointment.
There may be another patient scheduled after you, and your visit may be shortened if you join the call late.
If you are too late to begin the visit, we will have to reschedule you, and charges for the missed appointment will still apply.
We ask patients to be ready and logged in to the virtual waiting room at the time of their appointment so the doctor can begin the visit promptly.
